D&I Models in Health Webtool: Instrument Submission Form
Using this form, you are able to provide up to ten assessment instruments that you suggest we add to the Assess assessment instrument repository on the D&I Models in Health Webtool. If you would like to submit more than ten, please submit an additional form using the same link. 

Please consider the following criteria:
  • Needs to have direct relevance to implementation science
  • Assesses implementation outcomes at level of delivery staff, implementers or organizations rather than individual ‘recipients’ or patient level measures (e.g, clinical outcomes, quality of life, etc).
  • Diverse in terms of the type of the instrument - we are looking for instruments to meet the following types: SURVEY; INDIVIDUAL INTERVIEW; FOCUS GROUP INTERVIEW; PERIODIC REFLECTION; OBSERVATION GUIDE; DOCUMENTATION GUIDE; EMR FORM; OTHER (SPECIFY). 
    • We currently have 43 Qualitative Instruments; 15 Qualitative Instruments; and 5 Instruments that are both Quantitative and Qualitative

Please note: We cannot guarantee that the assessment instruments you submit will all be added. However, we will review each submission using our rigorous review criteria to determine which assessment instruments should be added. You can view how we select the assessment instruments here.
