The University of California, San Diego Department of Urology is conducting a research study to find out more about patient experiences after receiving treatment for testicular cancer. The results of this study will develop valuable insights into patient experiences and inform further research in testicular cancer treatments.

If you agree to be in this study, the following will happen to you:
You will complete a survey asking information pertaining to your demographic information, treatment course for testicular cancer, as well as physical, social, and psychological outcomes after treatment. This survey will also ask about sexual health and alcohol use. No further action will be necessary. The survey will take about 20 minutes to complete.

You will be asked to download an app from the Apple App Store or Google Play Store on a mobile device. Alternatively, you can watch a video on YouTube for the same information. You will then be asked to provide feedback on your experience with the app or video.

Research records will be kept confidential to the extent allowed by law. No personally identifiable information will be collected from you during the survey. Information will be stored on Qualtrics servers and deleted after approximately 18 months.

Participation in research is entirely voluntary. You may refuse to participate or withdraw at any time without penalty or loss of benefits to which you are entitled.
