Thank you for your interest in contributing to the exciting research conducted at the University of California San Diego (UCSD) Comparative Cognition Lab!

Every discovery we make and finding we are able to share is thanks to an amazing community of pet parents both near our home base of San Diego County as well as all around the world. Of course, we truly couldn't do it without the help of pets like yours!

This survey is the first step in joining our global community-driven citizen science initiative that uses Augmentative Interspecies Communication (AIC) devices to investigate communication between pets and their humans in an exciting new way. You should also fill this survey out if you are interested in participating in one of our other projects, including fun, in-person studies where you and your dog will visit our lab on the UC San Diego campus to contribute to cutting edge science that will feel like a game to your pup and earn them scritches and treats from our awesome researchers!

You will only need to fill out this survey once for any current or planned future Comparative Cognition Lab (CCL) study of animal cognition and communication, including in-person or remote studies.

Any personal information you provide (name, e-mail, postal code) will be securely stored and shared only with members of the team as required for research purposes. Please keep in mind that you can withdraw your pet and your contact information from our studies at any time and that the submission of this form does not obligate you to participate in any way, nor does it guarantee your pet's placement in any study.

This survey should take you 5-10 minutes to complete.

Our studies have been approved by the UCSD Institutional Animal Care and Use Committee (S21099).

I, the owner (as identified below), certify and consent to the following:

I am 18 years of age or older.
I am voluntarily participating with my pet, I understand I will not receive any monetary compensation for my participation, and I agree that research data provided by me or with my permission during the project may be included in a thesis, presented at conferences and published on the condition that neither my name nor any other identifying information is used.
I understand that even though I have volunteered myself and my pet to be involved, I can withdraw from any study at any time and can withdraw my data for any reason for up to four weeks following the completion of my pet's participation in the research. Furthermore, when withdrawing I can request that no information from my involvement in the given study be used.
I am aware that while the risk to my pet while participating in a study is minimal, there are some potential hazards. These include fatigue, distress, injury or illness from any activity conducted during the study, and injury to myself or other person(s) present if my petresponds with distress or fear during a study. I will take every precaution to minimize any and all potential risks.
I acknowledge that UCSD, the Principal Investigator, research students, and staff collaborating or associated with this research project have taken every precaution to minimize any risk to my pet, but they are unable to guarantee that participation in a study will be risk-free. I acknowledge that UCSD accepts no liability or responsibility for any distress, fatigue, injury, or illness sustained to me, another person, or my pet as a result of participation in any study.
